Thank you for participating in the PX3 Referral Program. Please review the following terms and conditions to understand how the program works and ensure eligibility for referral bonuses.
Eligibility Requirements
Referrer Eligibility:
The referrer must provide accurate contact information when submitting a referral.
Referrers can participate as individuals or as organizations.
Referred Candidate Eligibility:
The referred candidate must be successfully placed in a position and start employment with the referred organization.
Only one referral bonus will be paid per referred candidate.
Qualified Positions:
Referral bonuses are available for the following roles:
- Physicians: $6,000
- Advanced Practice Providers (APPs): $4,000
- Executives: $5,000
- Locums: $3,000
Referral Submission Guidelines
Referrals must be submitted through the official referral form on our landing page.
All required fields on the referral form must be completed accurately.
The referrer is encouraged to notify the referred candidate about the submission to ensure timely follow-up.
Bonus Payment Details
Payment Conditions:
Referral bonuses are paid only if the referred candidate is successfully placed and begins employment.
Payment will be processed upon the referred candidate’s actual start date.
Payment Timeline:
Bonuses will be paid within 30 days of the referred candidate’s confirmed start date.
Payment Method:
Referral bonuses will be paid via check, direct deposit, or other agreed-upon methods as determined by PX3 Medical.
Disqualifications
A referral will be disqualified if:
The referred candidate declines the position or fails to meet onboarding requirements.
Incomplete or inaccurate referral information is provided.
The referrer attempts to claim bonuses through fraudulent or unethical means.
Program Changes
PX3 Medical reserves the right to modify, suspend, or terminate the referral program at any time without prior notice.
Any changes to the program will not affect referrals submitted prior to the effective date of such changes.

By participating in the referral program, the referrer agrees to these terms and conditions.
Referral bonuses are subject to applicable taxes and reporting requirements. PX3 Medical will issue the necessary tax documents for bonuses exceeding applicable thresholds.
